The East Elgin Family Health Team is an interdisciplinary team of health care providers delivering primary health care services with a focus on chronic disease management, pre and post natal care, geriatric and palliative care and mental health programs.
We have a patient base of approximately 10,000 patients. Our team includes family physicians, nurse practitioners, registered practical nurses, a social worker, registered dietitian, psychiatrist and administrative support staff.
