The East Elgin Family Health Team is an interdisciplinary team of health care providers delivering primary health care services with a focus on chronic disease management, pre and post natal care, geriatric and palliative care and mental health programs.
We have a patient base of approximately 10,000 patients. Our team includes family physicians, nurse practitioners, registered practical nurses, a social worker, registered dietician, psychologist, psychiatrist and administrative support staff.
Our centres are located in the eastern region of Elgin County in the towns of Aylmer and Straffordville:

The closest major city is London, approximately 45 kms away followed by Tillsonburg, approximately 30 kms away and then St. Thomas, approximately 20 kms away. Hospitals are located in each of these cities.

Dr. Toogood was born and raised in St Thomas, ON. She graduated from the University of Western Ontario with an Honors Bachelor of Science in Physiology and Psychology and a Master of Science in Neuroscience. She completed her Bachelor of Medicine and Bachelor of Surgery at Flinders University in South Australia. Dr. Toogood completed her residency in Family Medicine at McMaster University.
Dr. Michael Fernando
Dr. Fernando obtained his Medical Degree and Bachelors of Neuroscience at Queen Mary and Westfield College, at the University of London, England. He also has a Master’s in Public Health from Imperial College, London, UK. Dr. Fernando currently practices at the EEFHT and in the Emergency department at Four Counties Hospital.
Dr. David Graham
Dr. Graham graduated from University of Western Ontario in 1982. After his residency he has been practicing in Aylmer since 1984. He loves woodworking and refinishing antique furniture.
Dr. Anne Howe
Dr. Howe was born in Brampton and moved to Aylmer when she was 16. She graduated from the University of Toronto in 1980. After she completed her internship she started practicing in Aylmer and she has since been practicing for 34 years. Dr. Howe has an interest in Palliative care and enjoys reading.
Dr. John Tokarewicz
Dr. Tokarewicz was born in Marmora, ON, a small mining town just south of Algonquin Park. He received his Medical Degree, CCFP in Family Medicine and FCFP from University of Western Ontario. He has been practicing family medicine for 31 years.
Dr. Charlotte Taylor
Dr. Taylor grew up in London and Port Stanley and was a Police Officer in Waterloo Region for 12 years. She attended the University of Waterloo and attained a degree in Liberal Arts. Dr. Taylor attended Michael G. Degroote School of Medicine and completed her Family Medicine residency at the Northern Ontario School of Medicine. She enjoys jogging, Taekwondo and spending time with her 2 sons.
Tracy studied at McMaster University and graduated with a Bachelor of Science in Nursing. She has worked in long term care homes, hospitals and the community. Tracy graduated from the University of Windsor where she completed her Primary Health Care Nurse Practitioner certificate and Master of Nursing. She has worked as a Nurse Practitioner at the EEFHT since 2009.
Janet received her Bachelor of Science in Nursing from York University in 2006. She returned to school in 2014 and graduated from Lakehead University with her Nurse Practitioner certificate. Janet has spent most of her career working in the Emergency and Post Anesthetic Care departments.
Michael Andrews is a life-long resident of Bayham. He is a telecommunications professional with extensive management and board experience in both the private and public sectors.
Abe Harms lives in Aylmer, is married and has two children and three grandchildren. He has been farming, in the area, since 1966. He grows mostly tomatoes and cucumbers.
Christie is a lawyer at Doyle Prendergast where she practices real estate and corporate law. She joined the EEFHT Board of Directors in November of 2015.
Mike was born and raised in Elgin County. He attended Wilfred Laurier University from 1986 to 1990. He serves a variety of clients and is licenced to perform audits, reviews, compilations and personal taxes.
Kent has lived in the Aylmer area most of his life with short stops in Lindsay, ON, Truro, NS and London, ON. He is the owner and board member of xiBOSS Corporation, an IT Staffing Agency, which does business in 29 countries around the world. He is honoured to be an EEFHT Board Member and views it as a way of giving back to a community.
Lew is a lifelong resident of the Municipality of Bayham. He is retired from the London Health Sciences Centre where he was a Planning & Facilities Coordinator. Lew serves on many different boards and committees within Ontario. He enjoys motorhoming and travelling abroad.
Shirley was born and raised in the Municipality of Bayham. She is a retired Registered Nurse and a Canadian certified Occupational Health Nurse.
Tina lives on a farm in the Straffordville area. She was a teacher and taught students in Port Burwell, Aylmer and Straffordville before becoming a Principal with the Thames Valley District School Board. Tina has an Honours Degree in Physical and Health Education from McMaster University. She is an avid curler and pickle ball player.
Jack has been a resident of Aylmer since 1970. He is a retired Principal of Aylmer’s East Elgin Secondary School and St. Thomas’ Central Elgin Collegiate. He has a Master of Arts in Soviet Studies and a Master of Education. He has served as an Aylmer Councillor and Mayor.
